The default options work pretty well for me (along with -r to go right to left). You can increase the size by setting dpi to 400 instead of 300, for example, e.g.
k2pdfopt -r -odpi 400 -vls 1.5 mydoc.pdf
Your margin cropping is a bit aggressive (it will cut off some of the text on some of the pages) and not really necessary--nor are the -ac or -de options since your scan is very clean.
The font varies in size because some of it is from the footnotes, which are typeset in smaller font.
The -vls 1.5 should keep the line spacing more uniform at 1.5x single space.
