That says the device hasn't processed these files and updated. Either the disconnect step hasn't been done since the files were put there, or the names are wrong.
But, I just had a look at the upgrade code. If you are upgrading to 3.11, then remove the md5sum file. As there are no kernel files, it isn't needed and might be blocking the update.

And I seem to be stuck here and actually can't do any reading with it until it's set up. So it looks like I actually do need to set up an account? or is there some work around?

From memory, firmware 1.9.11 should be usable without an account. You can't use WiFi or the shop, but you should be able to sideload books. It's been a while since I tried this. I'm sure the was an option to skip the setup that allowed you to continue. But, it wasn't obvious that the option would do this.
