I should have said that the output EPUB file was viewed on a PC using Edge. I transferred the EPUB file to my Android phone and it was much better. It's pretty good in Moon+ Reader Pro and slightly less perfect in Cool Reader. For example, links don't work in Cool Reader.
I'm surprised in the variations among EPUB readers. Is this common for "commercially" published documents? Or am I just seeing defects in the Calibre conversion?
