The comments around the configurable values in the 'adjustable top/bottom margin patch' should help you understand how those two patches can interact with each other .

EDIT: And as mentioned in the full message I quoted, top/bottom margins are now *fixed* in the kePub reader, so all this interaction talk is fairly moot if you never even read plain ePubs .
