For Kindle or actually tablets too you might try a stylus. To make it easier to hold you can get various foam pen/stylus holders that make it easier to hold in your hand with minimal finger effort.

I rest in bed and have no money left, however my headphone has 1 play/stop button and I succesfully set it up as a 2D X-Y selector in Android (Huawei ?) "Enhanced Accesibility".
Is not handsfree but I can push the button with any finger warm and rest under the quilt, no further need to raise my hand out for the screen. Works for me !
Continous scrolling might have been nice but seems to be available only for iOS.
