In the driver, is "kepub" in the list of formats the device supports? And is it above "epub" in the list?
In the settings for the KoboTouchExtended driver, is "Enable Extended Kobo Features" checked? If not, you probably want to check it.
Is there DRM on the books? If so, do you have "Upload DRM-encumbered ePub files" or "Silently Ignore Failed Conversions" checked?
That shows that if you have kepubs in the calibre library, they will be sent But, the other options are on the fourth tab of the dialog.
