I have not reported the issue to Kobo yet. There will hopefully be a new firmware today or tomorrow for the official release of the Forma and if the issue continues after that I will try and get it exchanged for a new one.

I looked it up and you are right (not surprisingly as you actually have the device
), the switching off auto-screen-rotate that I remember reading about is actually the 'lock orientation' -- but I didn't know that only disables rotation between portrait & landscape mode, leaving auto-rotation between left & right portrait active.
It's weird others don't seem to have run into the freezing issue... makes me think it's an error in your device's sensor (drivers) or something. In any case I hope it gets solved for you with the update!
