Thanks as well. Actually stumbled on to this solution in fixing the repeating cover problem by deleting the specific code, as suggested by ducks. As a second problem, if you want to remove the default cover supplied by calibre (i.e., to create an epub of text only) deleting the first HTML file in its entirety will do it.
To REMOVE existing covers

Modify EPUB plugin has a section just for covers. (As I mentioned before, the second appearance was just an IMAGE. To be a 'cover', Both the PAGE and the IMAGE file need to have been flagged as a cover. That is why this PI would not work for your old problem)

To STOP covers being added in Future CONVERSION to EPUB

Preferences: Output Options: EPUB (side bar): No Default cover (no tick)

THIS AFFECTS ALL EPUB CONVERSIONS.
You can also change this on a PER BOOK basis, by adjusting the Conversion Dialog that comes up at the start of all conversions
