I've made the changes locally to add a new flag:
-N, --no-original-toc "Default is to include the TOC from each original epub."

This seems to do what I was after.

If it is something you think might be generally useful, let me know and I'll submit a patch or pull request, whichever is easier for you. If not, no problems.
