Bit of a mistake in the book - Chapter 9 relies on an id of #09 but that doesn't exist. So the TOC and Contents Page are broken.

Minor issue, soon fixed, I'm at a loss about how it has happened as both the TOC and Contents page have the references to Chapter but there is no id associated with it for them to refer to.
