It's been a known issue for a long time that some device Kindle apps sometimes clone themselves after the app is updated. So always keep an eye on the Devices registered at Content & Devices.
I don't know if it still happens, but it wasn't unusual to end up with those clones, and that could play havoc with the simultaneous licenses permitted.
