If books are working on the second try, then there is something else happening. Unless you change something in the book, it should fail again. Opening the book in the calibre editor, checking for errors and fixing them should be enough to sort them out. Doing a conversion will usually sort it out as well.

Well it does work without changing anything, and it has worked that way for years. For the very rare instance when it hasn't, I did convert to solve the problem.
