OK, from what you have written, you have working USB only when you factory reset device. After you install KSM, USB stops working and this prevents you from working with it. But it gets solved again if you factory reset device.

Really not sure, but you may try older versions of KSM (7 and 8), also if it solves problem, you can report it here, it may help tshering to diagnose what's going wrong with your device.

But, you could copy koreader before instaling KSM in adds directory, so after installation you would have it (I guess) and you could use WiFi for file copying, etc. (not sure which options you have exactly, as I use USB for that).

First I thought you have some hardware problem, but if factory reset restores your USB, it looks like something software, or maybe software&hardware related.
