1) there are conversion settings for many formats. one of these is 'include TOC'. another is 'Put at end'. These are Per Format

The setting in Preferences is the DEFAULT for FIRST TIME conversions

If the book has been converted, the settings that were used ARE REMEMBERED (allows for custom, per book adjustments). You can change just that and reconvert

There is a button (tick box, in bulk mode) that causes these to be forgotten.
Polish and the Modify EPUB plugin allows you to do some cleanup without conversion.
