Looking at your log file, I'd suggest opening the epub in calibre's editor, click on the check book icon (looks like a beetle with an exclamation mark on its back) and fixing any errors (you are likely to also get warnings, don't worry about them for now). Any epub that triggers an error that "Root tag is not <html> in the XHTML namespace" has severe issues.

As usual, make sure you have a backup copy of the epub before modifying it.
