Output Options only affects ebook conversions.

I assume you're using Save to disk? You need to go to Preferences => Saving books to disks. Uncheck Save cover separately and Update metadata in saved copies.

If you wish to update metadata in the epub file but don't want to add a cover, use Polish books or the Modify ePub plugin before you run Save to disk.
