Could you maybe just shutdown your kobo to reboot? Maybe that's all that's needed to show the correct entry (I remember something similar when I first installed some additional dictionaries).

Apart from that, I've compared my entries to yours. They're almost the same: I've used "Русский - English" instead of "Russian - English" but that shouldn't matter. My Russian-English dictionary is a different one, it's smaller (no problem either, I'd suppose). I still have the original -it-en dictionary and for patching I used a non-break space instead of _ to replace the space after "Extra: " so I get an "Extra: ru - English" entry in the menu.
