Du müsstest einmal in die toc.ncx schauen.
Zum Beispiel:
"Gottfried Benn" verweist da auf "Section0003.html"; es fehlt das "#" plus die Überschrifts-ID:

Section0003.html#sigil_toc_id_27

wie es im inline-toc auch aufgeführt ist.
