Bookerly-BoldItalic.ttf The shorthand filenames used by k9reeder, above, are also supposed to be supported but I've never tried this naming convention for any of my own fonts.
If it isn't a simple filenaming problem then I'd need to see your 4 files. You can't post them here but if you zip them up, store them somewhere online (e.g. Dropbox) and PM me a link I could take a closer look if you want.

Well, in the end I both followed your suggestions and renamed the files as k9reeder suggested:
Bookerly.ttf
Bookerly-b.ttf
Bookerly-i.ttf
Bookerly-z.ttf
Then I powered the Kobo off and back on. Now Bookerly seems to be working fine (the book I'm reading doesn't have lots of italics, so I was only able to look at the italics in the TOC).
In the end, I suspect powering the device off and on was what made it work. Which makes me wonder if doing that after following your advice would have fixed it as well.
