What is OP?

As far as I remember I did hardly mess with Calibre library files, except maybe one or two cover.jpg I deleted to see if they would now show up with cover:false. I think I also may deleted some duplicate books (entire directory of the duplicate book).

Thanks for pointing out Library Maintenance. Check library found hundreds of missing covers. I click fix. Now I have >600 books with cover:false
