Preview.app on MacOSX can do a lot with pdf.
Ghostscript can translate from pdf to pdf and meanwhile do "stuff".
PDFEdit (
http://pdfedit.petricek.net/) can do quite some magical stuff with the innards of pdf.
PDFLab, sed/awk, vi whatever.
