One suggestion is to try what JSWolf suggested and put on one book at a time to see if you can locate the book that has issues.

On the other hand, I downloaded all 12 of the commented versions, ran them through epubcheck which was not overly happy (mostly due to the & # 13s inside the <pre> tags>. Also they claim to be epub3 but the declaration for the cover image in content.opf was an epub2 version. After I corrected the various errors (nicely consistent so I could simply run the same changes on each epub) and then uploaded them to my KA1, they processed happily enough. The cover images didn't show on the KA1 until I used Kobo Utilities to upload them and did a power cycle to clear the cached first page of the book as cover. Perhaps using the first file in the text area to display the cover image would have been a better idea...
