Converting PDF to ePub/MOBI/LRF

So I have this nicely designed PDF version of my online magazine...here's a link to a sample:

I used Zamzar to convert the PDF to html.

I've been using Calibre, and coming close...but not quite, to creating readable ePub/MOBI/LRF files.

Here's some of the main problems:
The page headers appear in the middle of the text and really junk things up.

The text of the stories are poorly formatted.

I'm sure these are common problems. Are there any quick tips to be given in regards to editing the html to make the conversion cleaner?

I'd be happy to share the html if someone enterprising mind wanted to donate their time to help.
