Yes, there is a utility available for those with a jail broken Kindle.

It is LibrarianSync - available in the "Kindle Developer's Corner" sub-forum.

It will create the Kindle collections from several sources -
From the directory layout below /documents -
From the file output by Calibre collections extension -
And it will take a manually created file.

But the thing is, you must jail break the device before it updates to the most recent firmware.
Put it in "airplane mode" and then check out the things above in the sub-forum.
That way it will not update while you are deciding what to do.
