Calibre keeps a copy of the cover for display in the GUI as not all formats support covers. The .opf file is kept to allow for recovery when/if the Main Calibre metadata database gets corrupted.

Thanks for your quick reply, but I'm not sure I was comprehensible (sorry I'm french).
I modified the comments and changed the cover at 21:21, but the epub last modification didn't change:

A the importation, Calibre created a cover.jpg and a metadata.opf from the epub (you're right, they are in the epub) but there are the old ones.
I would like to distribute the epub with the new cover and comments.
