What is the "normal" behavior when the Kobo recognizes the filesystem? Should any books on the SD card appear in the "normal" library, or perhaps as their own shelf?

I'd recommend using the SDCard Association formatter
SD Card Formatter 4.0 to format your card, preferably outside your Kobo using a SDCard slot or adapter to connect it to your computer. During the format, give the card a useful name since some version of the Kobo firmware had issues with nameless uSD cards. I would also recommend creating a directory on the card and building your library inside it to halp separate your files from other files/directories that may be created on the card.
Normal behaviour would be to scan the card for any new content after disconnecting from your computer and, if new content is found, import the content into your library. You can also check in the Settings/Device Information page where you should see both the internal and external storage listed.
