It is referring to two different things.

Refreshing the webpage just loads the latest version of the library content the server is serving, which may be the same, or may include database changes you have made.
The reloading the site popup means update the javascript that powers the offline reader. This happens whenever you update to a new version of calibre, or pull new changes to your source checkout with Development mode -- assuming that the new calibre code contains updated *.pyj source files that got compiled into an updated version of the calibre-server javascript.
