I have used every secret sauce posted on MobileRead forums, and most of them only resulted in the book's opening in the middle of the TOC sigh. Finally I put the "text" semantic (in Sigil) at page one, chapter one, and that worked to open it there.
More recently I have experimented with removing the semantic altogether. Now about three-quarters of my books open at the cover, both on the Look Inside and when opened on a Kindle device or app. I regard that as the Senator George Aiken solution to a somewhat different problem: "Declare victory and get out." I'll accept 75 percent. Or as we used to say in the defense industry: "Good enough for government work!"
