How calibre stores its files is calibre's business and not open to any user control.

You should only be concerned with the filenames produced when ebooks are "exported" from cailbre using either Send to Disk or Send to Device. There are templates that calibre uses for creating the filenames of exported files. These use data in calibre's library database and ignore the book's internal filename and folder name. You can edit these templates in Preferences.
