If the ebook is charged already, then you would need to:
  1. Register at www.ebookwise.com to setup your Online Bookshelf for purchased ebooks from ebookwise or fictionwise. But the best reason for you is that you get the free lookup dictionary when you do this! Look here for more startup help.
  2. Get and install the ETI usb driver so that you can use your computer to connect to the Online Bookshelf and later used by "transfer" software to load your Personal Content or .imp ebooks here. See IMP software for a complete list of programs available for you to use.
  3. (Optional) Get and install eBookwise/GEB Librarian ($$$) or Impserve (free) to organize and deliver your ebooks to your reader.

If not yet charged, a couple of hours of AC adapter charge should get you started!

Also, are there any hidden $$ charges I should know about - monthly fees, etc.? I don't mind paying for the reader and the books, but anything else I might encounter?
None, after you have acquired it! I've had mine for over 4 years and only pay for new ebooks, when I feel the urge...
