It was the same book in the first post. If I sent it as a epub, the ToC had the chapter names, and if I sent as a kepub (using the Kobo Touch Extended driver), it didn't. Changing calibre's settings to always use the generated ToC seems to have worked for that, although the ToC now doesn't include all the sub-sections. I think that's probably a configuration option someplace, though.
