Power users/readers probably mind a great deal, and they spend a lot of time fixing what they don't like. They also spend time posting about it on forums.
The majority of people either out of ignorance or lack of desire simply deal with the annoyances, and just keep on reading. I am in this class.
As an IT guy, I know about CSS, but I don't take the time to fiddle with eBooks. I haven't even done much with Calibre other than converting all my Kindle books to ePub when I switched over to a Kobo reader.
There are a few ebooks I've checked out from the library recently with some things that are a bit jarring when I finish start reading, but they fade away once I get involved in the story.
