I have only found one ereader program that lets me configure page turning options to do this. Do I have options as to how the page turning (as in where to touch the screen, which buttons do what) on dedicated eink devices?

On the Kobo, when holding with my left hand, I just do a very small swipe with my left thumb. It works fine. Alternatively I reach a little and tap in the middle (I have it set so that the right 2/3 pages forward on tap).
It can also be configured so that the whole bottom third pages forward when tapped, and the top third backward.
