Thank you. That gave me the idea to run a batch script in Photoshop to resize extracted pages from a pdf using Acrobat, then reassemble them back in Acrobat (those are the tools I have).
I find it hard to believe that Calibre could not be made to add inside margins since it is already doing something similar with different headers and footers that can be adjusted with margins, which is great in itself.
In fact, I was just thinking how cool it would be to add a page border to the outside of the pages that was staggered for each chapter like manuals are. Just an idea.
