The first one is expected. When you first send the book to the device, it is added to the device list but the size isn't recorded. The device list will show the size as zero.

When you disconnect the device, any new books should be processed into the database on the device and hence in to the library. You should be able to find it in the library list as well as any collection you added it to. You say isn't appearing in the collection, but can you find it on the device in the library? If it isn't in the library, then something is going wrong with the processing of the new books.

When you next connect, the Kobo driver reads the database on the device for the books. It doesn't scan the drive, only the database. If the calibre device list doesn't show a book, but Windows does, it means the book wasn't processed properly but the device. Or the metadata the device extracted from the file is different from what you have in the calibre.

It sounds like the book isn't being processed properly by the device. I don't use CBR or CBZ's very often, so I don't know why they don't get processed. The simplest reason I can think of is that the file isn't a valid zip or rar file. It could also be because some other book didn't process. They are processed in a batch and if a book doesn't process properly, any books found afterwards are skipped. The only solution for this is to remove the books that don't get processed and add them one at a time until you find the problem book.
