Do you see the SD card in the Settings/Device Information? You should see a line similar to:
Micro SD: 8190 MB of 15185MB available.
If you don't see that line, your Kobo H2O is not seeing the uSD card.

Does your computer show the uSD card when you connect your Kobo to it?

If not, I would try AnemicOaks suggestion and re-format your uSD card with the SDFormatter utility. After the reformat, re-insert it into your Kobo H2O and see if it mounts.

If it is still not visible to your Kobo H2O, I'd suggest trying another uSD card.
