To insert covers, I just always use Sigil's Tools > Add Cover. This creates a cover.xhtml page + automatically adds the correct Cover metadata.

For images in the middle of your book, RbnJrg recently told me about this Sigil Plugin, InsertImageSVG:

It makes creating the typical SVG wrapper so much faster/easier.
