Calibre will only convert the ebook format on send-to-device if it doesn't have a compatible ebook format already.

If you updated your epub version with FFF, but didn't re-convert it to azw3, Calibre is probably sending an out dated version to your device.

FanFicFare has a couple options to help with that, one that deletes other ebook formats and one that automatically starts Calibre Convert on each downloaded/updated ebook:

I use a Kindle and I keep both options turned on.

For the auto-convert, you also need to the default Convert output format to azw3 in Calibre's Preferences.
