Ummm... That's puzzling. The "Med" and freefromtags lines work fine for me. If they don't work now, I would suspect you have another conflicting custom_columns_settings set in another section or something. Or the column #names don't match anymore.

The tags line never worked. custom_columns_settings only sets custom columns--it's right in the name. If you're getting 'new' in Tags, it's coming from somewhere else.

I'd need to see your full personal.ini to give anymore help. There's a button for getting a 'safe' version with user/pass obscured. Also, when posting ini code, use [code] tags and if it's large, [spoiler] tags around that.
