I think you mean you don't use a PC to charge, but rather a USB charger...

It drives me crazy. I'm used to just connecting the Kindle, and it shows up. Half the time I'm sitting there wondering why Calibre doesn't detect the reader ("did the USB connection break again due to some Windows update?!") until I remember that I have to confirm the connection on the reader after plugging it in.
(Yes, I know, if you don't connect the reader and cancel the dialog, you could probably read while the reader charges. I never do that.)

I don't think so, and all readers I've had have the same function: ask when you connect a device to a PC if you want to charge it, or use it as storage device.

I have the same. I got a cable, brand new, that simply won't charge my Android tablet (Galaxy Note 8.0, so an older device). It will charge the device it came with: my Yotaphone.
