I have a somewhat similar problem when uploading a validated epub to Lulu.com. For each of my (Sigil-built) files, I get this error:

ERROR: IOException: Unresolved external XML entity: 'http://www.w3.org/TR/xhtml11/DTD/xhtml11.dtd' in OEBPS/Text/Section0002.xhtml

I found a Jutoh forum that addressed just this problem, with the advice:

The problem is that Lulu can't cope with bookmarks in the NavMap (toc.ncx file). The solution is to use a NavMap that doesn't have bookmarks, and you can achieve this by checking the configuration option Force simple NavMap. This overrides whatever NavMap you might have created using the command Book | Build Table of Contents, and generates a simple, one-entry-per-section NavMap with no bookmarks. Your table of contents page is not affected by this.
Can something similar be done in Sigil or Calibre?

Thanks! -- NJ
