Big hint to reduce the confusion -
When you read something that seems to contradict what is written in the first post of this thread, check the date of what you are reading.
If it was written prior to the start of this thread, 7/4/16 -
Then you are reading history.

Yes, the contents of MKK are included in the 'hotfix' package.

Yes, you have to re-install the Mobileread after-market add-ins that required installation.
That is, anything distributed in files with the pattern: update_*.bin and that you had to use MrPI to install.
Plus -
If it seems 'broke' after an Amazon update, re-install it.

The only exception to the above is the Jailbreak (certificate) and the contents of the 'hotfix'.
That is, the stuff that bridges the certificate installation across image updates.

Keep in mind:
MrPI is a 'batch mode' installer.
Put everthing update_*.bin into /mrpackages at the same time.
Trigger MrPI.
You are done.
