I am glad you still like KSM. In order set coolreader to auto start:
Find in .adds\kbmenu_user\confoptions\ksm_ini_options.txt
Code:
ksmAutoselectoption=start_nickel
ksmAutoselectoption=start_koreader
and add below the following line (use a unix-style editor)

ksmAutoselectoption=start_coolreader
Then (in KSM home), select "configure" and set in the group [general] ksmAutoselectoption=start_coolreader
If you never used "configure", you might have a look at
this post. If you have trouble with it, come back here. Maybe there is some post that has a better general description of how configure works. I cannot find one now.
As for your problem with the sleep cover; KSM does not use it. You have to disable the usage of it in each application, nickel, KOReader, Pbchess/Coolreader, KOReader.
