The cover for an epub, is the first page. The device renders the first page as needed. That means that the cover image is always the correct size for the device. But, exactly what you see depends on how the first page is designed. If you get calibre to create the cover, it can create it so that the image fills the page. Or keeps it's aspect ratio properly.

For kepubs, the cover is an image inside the book and has to be marked as such. The extended driver and the kepup converter will do this. If there is no image in the kepub marked as the cover, the default text cover with the book title and format will be used.

You can also send the cover to the device when sending the book. The options to do this is in the both the main KoboTouch driver and the extended driver.
