The only way to completely eliminate the header from KePub books is to use full-screen mode, which also removes the footer. See
this post for how to enable the full-screen feature. An alternative is to sideload the ePub version of the book; as the ePub reader doesn't have a header.

You can repeat instructions 3-6 in the first post as many times as you want to change the patch settings or add new patches, as long as your device is still running the same firmware version.
