I just pulled out "Process Explorer" from Sysinternals, and found that nothing was written. So, I ran "calibre-debug.exe -g" and then I got prompted for a filename to save to.
It's attached, and gives Calibre 2.71 and beyond the same look as before 2.71.

Reading the manual,
Creating your own icon theme for calibre - the it appear that the only purpose of creating a 'theme' is should you want to make it available to the world at large via the
Look and Feel->Change icon theme feature.

You can upload that to the calibre forum at Mobileread and then I will make your theme available via calibre’s builtin icon theme system.

Otherwise whatever is in the
%CALIBRE_CONFIG_DIR%\resources\images folder is your personal icon theme. If you install a published theme its icons are written to that folder, I don't know if they will overwrite anything that was already there.
