Reading the manual, Creating your own icon theme for calibre - the it appear that the only purpose of creating a 'theme' is should you want to make it available to the world at large via the Look and Feel->Change icon theme feature.

Otherwise whatever is in the %CALIBRE_CONFIG_DIR%\resources\images folder is your personal icon theme. If you install a published theme its icons are written to that folder, I don't know if they will overwrite anything that was already there.
