If that is the cause of the problem then the easiest solution is probably to use Calibre to add a proper cover to the book.
I've found that adding a cover in Calibre may only add an image that is marked as the cover. Depending on the tool/plugin you use to update the ePub, it may not actually create an xHTML file as well. You may find that opening the ePub in Calibre's Editor and adding the cover there is more effective as that will add the image and create an xHTML file containing a link to the image as well.
