The oversimplified version ('cause I don't remember the exact details of how it all works ) is that if they have specified margins in the CSS of the book it can override your ability to change the margins on your Kobo. IIRC it's relatively easy to change that in Calibre without doing much work.
Why it happens is because basically the Kobo is following what the book is telling it to do. It has been extensively argued whether this is a good thing or a bad thing. It's not really a common thing to run into, but not really uncommon either (and happens in books from various sources).
If I have some of this wrong I'm sure those in the know will be along shortly with the correct answer .
