I have every box unchecked. I am very strict about what tags I use as well, but used to have to delete/change those. The ratings field is usually left blank when I load a new book.

I think what happened is that the author used Calibre to create the epub version of the book. Rating is one of the fields that Calibre stores metadata for when you do a conversion. And she so helpfully decided her book was great Or whomever she hired to help with formatting did so. I fixed the coding so that the text didn't not appear immediately below the words "chapter x" with no space between them and noticed some of the css had calibre in it.
