I had the same behaviour (on Windows 10) with a couple of other packages before I experienced it with Calibre. Evernote was one and maybe 1by1 or ConEmu was the other.

In all cases removing the existing pinned item from the TB, running the program from the .exe and pinning the 'transient' icon solved the problem - so far for all time.

@thed0g - are you running any taskbar enhancement gadgets, or have you applied any related tweaks. If so try killing/undoing it and see if that overcomes the problem. FWIW, I use 7+ Taskbar Tweaker with no ill effects.
